How is diabetes treated?

The major goal in treating diabetes is to minimize any elevation of blood sugar (glucose) without causing abnormally low levels of blood sugar. Type 1 diabetes is treated with insulin,exercise, and a diabetic diet. Type 2 diabetes is treated first with weight reduction, a diabetic diet, and exercise. When these measures fail to control the elevated blood sugars, oral medications are used. If oral medications are still insufficient, treatment with insulin is considered.

Adherence to a diabetic diet is an important aspect of controlling elevated blood sugar in patients with diabetes. The American Diabetes Association (ADA) has provided guidelines for a diabetic diet. The ADA diet is a balanced, nutritious diet that is low in fat, cholesterol, and simple sugars. The total daily calories are evenly divided into three meals. In the past two years, the ADA has lifted the absolute ban on simple sugars. Small amounts of simple sugars are allowed when consumed with a complex meal.

Weight reduction and exercise are important treatments for diabetes. Weight reduction and exercise increase the body's sensitivity to insulin, thus helping to control blood sugar elevations.

Medications for type 2 diabetes

WARNING: All the information below applies to patients who are not pregnant or breastfeeding. At present the only recommended way of controlling diabetes in women who are pregnant or breastfeeding is by diet, exercise and insulin therapy. You should speak with your doctor if you are taking these medications and are considering becoming pregnant or if you have become pregnant while taking these medications.

Based on what is known, medications for type 2 diabetes are designed to:

  1. increase the insulin output by the pancreas,

  2. decrease the amount of glucose released from the liver,

  3. increase the sensitivity (response) of cells to insulin,

  4. decrease the absorption of carbohydrates from the intestine, and

  5. slow emptying of the stomach to delay the presentation of carbohydrates for digestion and absorption in the small intestine.

When selecting therapy for type 2 diabetes, consideration should be given to:

  1. the magnitude of change in blood sugar control that each medication will provide;

  2. other coexisting medical conditions (high blood pressure, high cholesterol, etc.);

  3. adverse effects of the therapy;

  4. contraindications to therapy;

  5. issues that may affect compliance (timing of medication, frequency of dosing); and

  6. cost to the patient and the healthcare system.

It's important to remember that if a drug can provide more than one benefit (lower blood sugar and have a beneficial effect on cholesterol, for example), it should be preferred. It's also important to bear in mind that the cost of drug therapy is relatively small compared to the cost of managing the long-term complications associated with poorly controlled diabetes.

Varying combinations of medications also are used to correct abnormally elevated levels of blood glucose in diabetes. As the list of medications continues to expand, treatment options for type 2 diabetes can be better tailored to meet an individuals needs. Not every patient with type 2 diabetes will benefit from every drug, and not every drug is suitable for each patient. Patients with type 2 diabetes should work closely with their physicians to achieve an approach that provides the greatest benefits while minimizing risks.

Patients with diabetes should never forget the importance of diet and exercise. The control of diabetes starts with a healthy lifestyle regardless of what medications are being used.

    How Can I Protect My Eyes from the Computer?

    Computer-related eye strain is a common workplace complaint, as many employees spend full shifts in front of acomputer without a break. In addition to causing eye strain which can lead to squinting, dry eyes, and general discomfort, prolonged periods in front of a computer can lead to other physical stresses as well, if the environment is not ergonomically arranged. Always set up your workspace to protect your eyes, along with the rest of the body, because it will keep you healthier and more productive. There are a number of steps you can take to protect your eyes so that you will not feel strained and tired when you arrive home from work.

    To begin with, make sure that your workspace is arranged in a way that will protect your eyes. The ambient light level should be low and diffused, with no glare. Keep windows to the side of you, rather than directly ahead or behind, to minimize glare. Also adjust the brightness of your computer screen to protect your eyes; your screen's lighting should match the general light level in the room, without being too bright or too dim. Try bringing up a blank word processing document and looking at the screen. If the white of the page is glowing, your screen is too bright. If it is dingy, the screen is not bright enough. Also set up your desk so that your screen is approximately 20-26 inches (50-66 centimeters) away from you, and slightly below eye level. If you work with paper documents, protect your eyes by keeping them at the same level as your computer screen, so that your eyes do not have to constantly readjust.

    During your working day, protect your eyes by looking away from the screen to a distant point approximately every 10 minutes. Every half hour, take a full break, stepping away from the computer to walk and stretch. This will protect your eyes and your body, preventing you from slumping into poor posture which may lead to back pain and repetitive stress injury. Make sure to blink frequently, and use artificial tears or eye drops if your eyes start to feel dry. If the screen is hard to read, try resizing the text or changing the resolution to make it easier to read.

    You should also protect your eyes by getting an annual eye exam if you work with computers. During the exam, an optometrist can check on the health of your eyes and talk with you about ways to protect your eyesat work. You may, for example, need a special glasses prescription for computer work. An annual eye exam will also catch any serious problems with your optical health early.

    Taking Care of Your Ears

    As body parts go, your ears don't ask for much. They don't need to be brushed like your teeth or trimmed like your toenails. All your ears need is to be washed regularly, so wash them with soap and water while you're sudsing up the rest of your body in the bath or shower.

    The Facts on Earwax

    You might wonder about earwax and whether it needs to be cleaned out. Actually, even though earwax seems yucky, it serves a purpose. Your ear canal makes earwax to protect the ear. After it is produced, it slowly makes its way to the opening of the ear. Then it either falls out or is removed when you wash. If you want, you can clean the opening of your ear gently with a washcloth.

    There's an old saying that you shouldn't put anything smaller than your elbow in your ear. Not all old sayings are true, but this one is! You can hurt your ear by poking around in there.     Pierced ears may look pretty, but you need to take good care of them or things can get ugly! When you first get your ears pierced, leave the earrings in until your ears are completely healed. If you don't, your holes could close up.

    You'll also want to prevent infections in your newly pierced ears. Wash your hands before touching your pierced ears. Applying rubbing alcohol also can help keep germs away. With an adult's help, soak a cotton ball in rubbing alcohol and apply it to both sides of the hole (with the earring still in your ear). Then twirl the earring several times to make sure the alcohol gets in and around the earring post.

    If you think one of your pierced ears may be infected, tell your mom or dad. An infected earlobe may be swollen, red, warm, and painful, and it may ooze a fluid called pus. Don't wait for it to get better by itself because the infection may spread and make you sick.

    Sometimes, swimming can lead to a case of swimmer's ear. That's when your outer ear gets infected, causing swelling and pain. This happens when water gets stuck in your ear canal. Special eardrops can help you get rid of it.

    To prevent swimmer's ear, dry your ears after swimming and shake out excess water, especially if you feel it stuck in there. Again, with your parent's help, a few drops of rubbing alcohol can dry out any moisture in there. Drying your ears will keep you — and your ears — in the swim!

When the sun is beating down, wear a hat or remember to rub some sunscreen on your ears, so they don't get fried. And when winter rolls around, keep them covered up with a hat or headband. Why?
